  • Description

    The Nurse Care Coordinator in the Pediatric Hematology is responsible for providing direct and indirect clinical care to pediatric outpatients who have a variety of hematological disorders. Care is provided through the systematic application of the nursing process: assessment, planning, implementation, and evaluation. All components of this process are individualized to meet the physiological, psychosocial, and developmental needs of the patients. The Pediatrics Hematology Nurse Care Coordinator performs ongoing assessments and modifies the care planning process as needed, to ensure continuity of care between inpatient, outpatient, homecare, home, and multiple providers. The Pediatric Hematology Nurse Care Coordinator is actively involved in patient, staff, and provider education. (S)he participates in clinical research activities within the program and incorporates research findings into planning and implementation of care.
