    We are looking for a full time Registered Nurse - On Call to join our hospice team.

    Multiple scheduling options available:

    • Friday, Saturday and Sunday 7p-8a every weekend

    • Thursday, Saturday, Sunday 8a-9 every weekend

    • 7 days on, 7 days off

    • On-call Triage Unit will help take care of easy fixes and forward to you the critical situations.

    • Traveling to patient homes to handle emergent issues including new admissions, deaths, and other urgent matters.

    Reports directly to the Administrator, Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family and hospice team.

    • Provide direct patient care services to hospice patients and supportive care to the patient's family after normal business hours on an on-call basis

    • Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care

    • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient

    • Maintain all assigned patient clinical records

    • Participate in company quality assessment and performance improvement programs

    About You

    • Compassionate, kind and empathetic to your patients and their families

    • A current RN license (in the state of requested employment or ability to obtain)

    • 1+ years of nursing experience in hospice, home health, or oncology preferred
