  • Children's Hospital of Pittsburgh is hiring a Pharmacist to work in the inpatient Pharmacy and the main hospital location in the Lawrenceville section of Pittsburgh, PA.

    The Pharmacist will ensure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research.

    Responsibilities:

    • Review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medications.

    • Identify and report suspected adverse drug reactions accurately and in a timely manner.

    • Interpret and evaluate physician orders in order to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the Practicing State's Pharmacy Practice Act, Federal Drug Laws, and other regulatory organizations.

    • Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area.

    • Organize daily operational duties and assist in the supervision of pharmacy technicians, pharmacy aides, student interns and other ancillary personnel involved in providing pharmaceutical care.

    • Fill prescription orders in accordance with State and Federal regulations, utilizing technological resources and equipment.

    • Utilize all available technologies in the provision of pharmaceutical care (i.e., computer systems and databases, Acu-Dose, Robot-Rx, pneumatic tube system, etc).

    • Provide drug information and/or educational programs to healthcare professionals and pharmacy students.

    • Communicate with physicians and nurses to insure prompt clarification of orders that are unclear, illegible, or incomplete.

    • Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection.

    • Adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.

    • Upon becoming aware of a dispensed medication variance, ensures patient safety by notifying patient's physician and nurse in a timely fashion. If possible, immediately retrieves, or oversees retrieval of, incorrect product. If necessary, prepares and d

    • Where applicable, assist in the completion of monthly Nursing Unit liaison reports/emergency medication cart checks by the last day of each month following Department and JCAHO guidelines.

    • Audit and maintain accurate records for all controlled subtances. Assure that all controlled substances are stored and dispensed in compliance with Federal, State, and Departmental regulations.

    • Support drug-related research by assisting in the procurement, accountability, storage, preparation, and distribution of investigational/study drugs.

    • Screen all medication orders for Adverse Drug Reactions/Medication Errors as per Department standards and report through appropriate reporting process.

    • Where applicable, document clinical interventions in accordance with Department procedures.

    • Review medication orders for safe and effective indication, dose, route, and scheduled administration. This monitoring and evaluation is appropriately documented.

    • Responds to arrest codes and participates as required.

    • Annually complete continuing education programs related to Pharmacy Practice as a mandatory requirement for maintaining an active license in the Commonwealth of Pennsylvania.

    • B.S. degree in Pharmacy or Doctor of Pharmacy (Pharm D.) from an accredited school of pharmacy required.

    • 3-6 months hospital pharmacy experience is preferred.

    • Previous experience or familiarity with investigational drug control procedures and protocol design is preferred. Licensure, Certifications, and Clearances:

    • Pharmacist

    • Act 33 with renewal

    • Act 34 with renewal

    • Act 73 FBI Clearance with renewal
