    JOB SUMMARY

    Supports a safe environment for patients by providing continuous observation and/or surveillance of assigned patients. Responsible for verbal redirection of the patient(s) from engaging in potential at risk behaviors, notifying appropriate professional staff of patient's needs including unsafe behaviors, and appropriate documentation on patients' medical records along with other documents, such as handoff report. Job duties include patient assistance with activities of daily living and may include other duties as assigned.

    PRIMARY JOB RESPONSIBILITIES AND DUTIES

    1. Maintains confidentiality of information pertaining to patients and their families, physicians, colleagues, and visitors to Trinity Health. Ensures appropriate privacy for patients.

    2. Provides care to patients including behavioral health population based upon patients' individualized treatment plans as delegated by RN.

    3. Demonstrates knowledge and skills to competently care for all assigned age groups (Child, Adolescent, Adult, and Geriatric as applicable).

    4. Completes assignment under the direct supervision of licensed nursing staff caring for patient.

    5. Accepts and provides direction in a cooperative and positive manner. Effectively communicates and uses discretion and courtesy when working with patients, visitors and staff.

    6. Collaborates with nursing staff on patient care and needs. Prioritizes tasks when needed and seeks assistance as needed.

    7. Provides continuous observation of patients as assigned and maintains continuous clear visual observation of entire patient including clear visualization of patient's face, neck and hands.

    8. Recognizes potential patient behaviors and actions that may place patients and/or others at risk, and verbally redirects patient in order to support a safe environment for patients and others.

    9. Participates in a collaborative identification and reporting of patient safety concerns or issues. Ensures patient environmental safety and seeks assistance with removal of any potential safety hazard from area and communicates to RN.

    10. Recognizes and responds quickly to patient behavioral or other changes; notifies nursing staff for patient assistance or for other concerns regarding patient. Seeks assistance immediately when patient appears to pose a threat to themselves or others.

    11. Responds to patients' needs including assistance with activities of daily living, movement, and seeks assistance as needed.

    12. Documents on patients' medical records and/or other required documents.

    13. Informs nurse prior to leaving assignment/and or for continuous observation replacement in order to provide continuity of care to patient(s) and maintain safey. Participates with handoff report with staff prior to assuming care of patient(s), and prior to leaving assignment.

    14. Able to work with patients in Isolation and maintain isolation precautions.

    15. Performs other duties as assigned.

    MINIMIUM QUALIFICATIONS, REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

    1. High School Diploma or equivalent. Previous experience in a hospital setting helpful.

    2. Basic Life Support for Healthcare providers (Red Cross or AHA).

    3. Interpersonal skills to effectively communicate with patients, visitors, staff, and others. Avble to maintain professionalism during stressful and difficult situations.

    PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

    1. Must be able to set and organize own work priorities and adapt to them as they change frequently.

    2. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles.

    3. Must possess the ability to comply with Trinity Health policies and procedures.

    4. Ability to concentrate with interruption and attend to tasks for more than 60 minutes at a time.

    REPORTING RELATIONSHIPS

    1. Reports directly to the Nurse Manager

    2. Has no responsibility for the supervision of staff.

    Position Purpose

    Minimum Qualifications

    • Education: H igh school diploma or equivalent. Previous experience in a hospital setting helpful.

    • Basic Life Support for Healthcare Providers (Red Cross or AHA)

    • Must be able to commit to 6 weeks of training/orientation

    • Interpersonal skills to effectively communicate with patients, visitors, staff and others. Able to maintain professionalism during stressful and difficult situations.

    • Ability to pass drug screening and background check.
