    As the Patient Care Specialist, you'll contribute to our success in the following ways:

    • Establishes and maintains effective and efficient systems for tracking program referrals to ensure accurate admission processes are followed.

    • Communicates program referral status to appropriate sales, branch, or administrative team members.

    • Answers all inquiries regarding enterprise services and provides appropriate information, including referrals to community resources as appropriate.

    • Actively works to connect patients with appropriate internal team members based on patient needs and requests, including but not limited to skilled care, personal care, ElaraConnect personnel or others as appropriate

    • Qualifies referrals for skilled care by assessing patients over the phone and in conjunction with available clinical documentation and patient feedback

    • Verifies eligibility for skilled care as part of qualification process, including but not limited to insurance eligibility, branch capacity, consent for care, and physician approval of home health order.

    • Collaborates with orders team to retrieve signed home health orders from referring physician or facility to ensure timely initiation of care.

    • Follows regulatory requirements and timelines when assessing new potential patients.

    • Initiates and maintains collaborative relationships with patients, caregivers, referral sources, and physicians.

    • Participates in clinical record audits, utilization reviews, Peer Review or Quality Improvement programs, as appropriate.

    • Attends required training, education, seminars, or other means of learning.

    • Processes all data entry information into the company database software accurately and timely.

    What is Required?

    • Associates Degree in Nursing in Nursing is required.

    • 2+ years' experience as an LPN/LVN

    • Current Certified LPN/LVN

    • Proficiency with MS Office Suite (Word, Excel, Outlook)

    • Prior experience with enterprise applications (e.g., Homecare Homebase, Workday, etc.)

    • Excellent verbal and written communication skills.

    • Experience with Medicare/Medicaid home health care benefits, policies and procedures is preferred.

    You will report to the Clinical Program Supervisor.
