    This advanced practice provider will join Community Health Network, a multispecialty group with over 1,300 providers in Indianapolis, IN. This advanced practice provider will be focused on the well-being of Community Health Network employees and their beneficiaries, at our Worksite Care Clinic conveniently located on our South hospital campus.

    Highlights

    Community Health Network has an excellent opportunity for a family medicine certified physician assistant or nurse practitioner to work at our Community Hospital SO Worksite Care Clinic, for employee health needs.

    • 4 Days; Occasional Saturday at Community Hospital North

    • Ages 5 and up

    • Practice hours are Monday - Friday 7:30-6:00 pm

    • Electronic Medical Record (EPIC)

    Job Duties

    Advanced Practice Provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

    • Routine check-ups and preventive care

    • Diagnosis and treatment of illness or injury

    • Managing chronic or complex health conditions

    • Medication management

    • Referral to specialists for more serious conditions

    Education

    • Educational background consistent with requirements for licensure and certification with the State of Indiana.

    • Provider must be family medicine trained and family medicine board certified.

    • Minimum two to three years of experience as a provider is required.
