    The BIDMC Nursing Mission is to build on a legacy of nursing excellence by caring with compassion, advancing the art & science of nursing, and advocating for the health of patients, families, and communities. The Nurse Case Manager provides utilization management, care coordination and discharge planning for an assigned patient case load. This role is patient-focused, outcome-oriented and based on a general and specialty professional standards, and functions within a multidisciplinary practice model. Training, evaluation and competencies for this role are overseen by a Nurse Leader to the department.

    Job Description:

    Primary Responsibilities

    1. Conducts concurrent daily chart review, investigating patient history, symptoms, clinical status, and treatment utilizing Interqual ISD criteria as a tool to determine a patient's severity of illness and the intensity of service. (essential)

    2. Works with multidisciplinary team to develop treatment plans, including contingency plans. (essential)

    3. Leads daily case management rounds on assigned unit. Participates in house staff rounds. (essential)

    4. Provides necessary information to appropriate organization to obtain continued stay certification; assesses payers' and referring physicians' needs in regard to content and frequency of communications. (essential)

    5. Facilitates early referrals to social work, rehab services, patient advocacy, and quality management as needed. (essential)

    6. Refers cases as appropriate to the Utilization Review Physician for review and determination of need for continued stay or alternate healthcare setting. (essential)

    7. Identifies clinical issues and lapses in standards of care and notifies appropriate parties. (essential)

    Required Qualifications

    1. Associate's degree in Nursing required. Bachelor's degree in Nursing preferred.

    2. License Registered Nurse required.

    3. 1-3 years related work experience required.

    4. Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.

    Preferred Qualifications

    1. Certified Case Manager.

    2. Previous clinical nursing experience and/or case management experience.

    5. Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.

    6. Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.

    7. Team Work: Ability to lead and direct multiple collaborative teams for large projects or groups both internal and external to the Medical Center and across functional areas. Results have significant implications for the management and operations of the organization.

    8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.

    Age based Competencies: Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 - over.

    Social/Environmental Requirements:

    1. Work requires periods of close attention to work with out interruption. Concentrated effort of up to 4 hours without break may be required.

    2. Work requires constant response to changing circumstances and using new information to adjust approach and to quickly respond to new needs.

    3. Potential exposure to adverse environmental conditions

    o Daily: Protective equipment required(Respirator,earplugs,mask,gloves,eyewear etc), Potential exposure to infectious diseases and/or airborne pathogens, Potential exposure to infectious diseases and/or bloodborne pathogens.

    1. Health Care Status: HCW1: Regular, day-to-day contact; both face-to-face and hands-on (having close contact within three feet for at least five minutes). Examples: physicians, clinical nurses, phlebotomist, medical assistants, PFT tech, and x-ray tech.- Health Care Worker Status may vary by department

    Sensory Requirements

    Close work (paperwork, visual examination), Monitor Use, Visual monotony, Visual clarity <3 feet, Conversation, Monitoring Equipment, Telephone, Background Noise.

    Physical Requirements

    Sedentary work: Exerting up to 10 pounds of force occasionally in carrying, lifting, pushing, pulling objects. Sitting most of the time, with walking and standing required only occasionally

    This job requires constant sitting, Endurance-working up to 3-4 hours without a break,frequent walking, standing, Keyboard use.There may be occasional Pushing/Pulling using both hands, Reaching-above shoulder height, Reaching-below shoulder height, Lifting and carrying items weighing up to 10 lbs, Lifting and carrying items weighing up to 25 lbs.Rarely there may be Fine Manipulation using both hands, Lifting and carrying items weighing up to 50 lbs, Lifting and carrying items weighing up to 75 lbs, Lifting and carrying items weighing up to or more than 100 lbs.
