  • The Home Health Aide is a member of the home care team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient's needs.

    Essential Duties and Responsibilities

    • Responsibilities of the Home Health Aide include, but are not limited to: baths; applying lotions to skin, oral hygiene, shampoos, changing bed linen

    • Assists patients with dressing and undressing

    • Performs skin care to prevent breakdown

    • Assists the patient with toileting activities

    • Keeps patient's living area clean and orderly, as appropriate

    • Plans and prepares nutritious meals

    • Assists in feeding the patient, if necessary

    • Takes and records oral, rectal or axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

    • Assists in ambulation and exercise according to the plan of care

    • Performs range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

    • Assists patient in the self-administration of medication per policy

    • Handles patient's laundry, as appropriate

    • Meets safety needs of patients and uses equipment safely and properly (foot stools, side rails, etc.)

    • Reports on the patient's condition and significant changes to the assigned nurse

    • Adheres to Amergis' documentation and care procedures and standards of personal and professional conduct

    Minimum Requirements

    • Certification may be required per state/contract

    • Complies with all relevant professional standards of practice

    • Participation and completion of Amergis' Competency program when applicable

    • Current CPR if applicable

    • TB questionnaire, PPD or chest x-ray if applicable

    • Current Health certificate (per contract or state regulation)

    • Must meet all federal, state and local requirements

    • Successful completion of new hire training as applicable to job site

    • Understand patient confidentiality and HIPAA requirements

    • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

    • Computer proficiency required

    • Must be at least 18 years of age
