  • Adecco Healthcare & Life Sciences is currently hiring a Case Manager in Kissimmee, FL .

    This is a full-time, on-site position.

    Pay: $45-$47/hour

    Type: Three month contract, possibility of an extension

    Schedule: Monday-Friday, 8:00 AM-5:00 PM

    Location: Kissimmee, FL 34741

    Primary Responsibilities

    • Conduct comprehensive patient assessments and collaborate with providers to develop individualized care plans.

    • Coordinate services and resources to meet patient needs, advocate for patients, and monitor progress.

    • Facilitate smooth transitions and discharge planning for patients.

    • Support ongoing evaluation and documentation of patient outcomes.

    Qualifications

    • Bachelor's Degree in Nursing (BSN) required; LPNs may be considered.

    • 3+ years of experience in case management.

    • Knowledge of community resources and strong critical thinking/problem-solving skills.

    • Positive, motivational patient engagement.

    • Professional certification required.

    Pay Details: $45.00 to $47.00 per hour
