Right now, we are looking for an experienced and dedicated Wellness Partnership Director. Full operational responsibility and accountability across all product lines (home health, therapy, physician house calls, mobile diagnostics, private duty). Within a designated facility location(s), ensure the coordinated delivery of all of AllianceCare services. Facilitate the support and integration of all service lines to meet the health and wellness needs of residents, families, and facility staff/leadership. Develop a well-informed and trained clinical team to achieve quality patient care through creative planning, supervision, counseling and evaluating the clinical and managerial performance · Manages Quality of Care outcomes. · Customer Relationship Management - ensuring AllianceCare's facility wellness partnership customers are satisfied with services. Including the timely resolution of all issues and complaints · Ability to fully understand the nuances and key drivers in their geographic area · Responsible for implementing the AllianceCare comprehensive, collaborative health and wellness model including monthly wellness programming within their designated facility location(s) · Responsible for completing and implementing quarterly strategic marketing plans for all designated facility location(s) · Responsible for encouraging teamwork and teambuilding and conducts regular meetings with the staff to keep them apprised of program and Company developments. · Responsible for implementing quarterly health status surveys for all residents in designated facility location(s) · Responsible for completing all monthly and quarterly client management reports · Hires, mentors, coaches and encourages the growth of future leaders · Ensures teams have adequate support and resources.
