Conducts a thorough and precise assessment of the patient's condition, functional abilities, homebound status, and environmental factors. Makes appropriate recommendations for necessary equipment, home modifications, and referrals to ST facilities and other therapies as indicated. Provides care in accordance with physician orders, the patient's condition, accepted standards of practice, and Agency policies, procedures and processes. Identifies changes in the patient's condition, consults with physician and/or Patient Care Coordinator (PCC), and implements appropriate interventions. Utilizes appropriate infection control techniques. Develops and updates an individualized speech therapy treatment plan to include education of patient's physical condition. Develops and implements a speech therapy treatment plan and discharge plan with the involvement of the patient and the patient care team. Qualifications: * Free from Medicare Sanctions (required) * Current ST license (required) * Current CPR certification (required) * Adequate means of transportation to facilitate timely visits (required) * Minimum of 12 months experience as an ST, with Home Health experience (preferred) E.O.E./M.F.D.V.
