Service Excellence Manager Department: Performance Improvement Category: Administrative Type: Full-Time Shift: Day Description: POSITION SUMMARY: The Service Coordinator is responsible for assisting the clinical staff in case management activities and the coordination of Program services with other service providers. MAJOR POSITION RESPONSIBILITIES: Acts as an advocate for patients by assisting them in obtaining resources such as housing, transportation and etc. Interacts with other agencies to establish professional relationships and determine what services are appropriate for Program patients. Develop therapeutic relationship with all patients; schedule opportunities for one to one interaction to review case management needs. Attends treatment-planning meetings to discuss service coordination and case management. Contacts/works with family members and other significant people who provide external support to patients. Document contacts with patients in the medical record describing focus on service coordination and outcomes. Provides milieu management as needed, “rounding up” patients to their groups in a timely manner. Provides back up to the Clinical Supervisor in writing the daily schedule/roster. Complete discharge audits of medical records per department standard. Works collaboratively with Transportation Dispatcher to insure appropriate scheduling of patient transportation. Provide assistance to Operations Manager, Clinical Supervisor, and Utilization Review specialist in preparing medical records for Medicare Additional Development requests and denials. Access relevant patient reimbursement data utilizing the Direct Data Entry (DDE) system. Determine patient Medicare eligibility for services utilizing the Direct Data Entry (DDE) system. Proactively support performance improvement efforts within the organization; identifies opportunities and participates in improving the quality of patient care and/or work processes. Participate in performance improvement projects as requested by the Operations Manager of Outpatient Behavioral Health. ---------------------------------------------------------------------- QUALIFICATIONS: Microsoft Word and Excel skills · At least one year of related clinical experience, preferably with individuals with serious and persistent mental illness or other serious disabilities. · Strong MS Office (including Word and Excel) skills · Strong case management skills · Knowledge of community resources and agencies · Familiarization with Medicare and MediCal procedures EDUCATION: Bachelor’s degree in Psychology, Social Work, Counseling, Behavioral Science or related field required.
