We are seeking an RN Patient Care Coordinator with a caring and giving spirit, ready to help the underserved in the Harris County Region access specialty healthcare services.
RN Patient Care Coordinator
Under the supervision of the Provider Health Network Manager, the RN Patient Care Coordinator provides assistance to uninsured and underinsured persons in need of specialty health services. The RN Patient Care Coordinator will coordinate patient care from referral to diagnosis to treatment, maintain active contacts with referral sources, provide ongoing orientation and assistance to new and established services providers, supervise health navigators as needed and assist the Provider Health Network Manager in facilitating referrals for ancillary, hospital and specialty services. Requirements
Qualifications: · Licensed Registered Nurse in Texas · 2-5 years experience in healthcare setting; prefer experience in case management · Supervisory experience preferred · Proficient using a computer with ACCESS database · Must have dependable transportation
