EDUCATION AND TRAINING

Degreed RN.                                             

LICENSING/REGISTRATION/CERTIFICATION:

Current RN licensee in the state of Missouri.

EXPERIENCE:

Two years previous experience with long term care plans, MDS, state regulations and HCFA

guidelines.

SKILLS AND ABILITIES:

Excellent communication skills to interact verbally with patients, visitors and staff.

Ability to manage time and organize daily schedule to meet productivity standards
Job Responsibilities:
The MDS Coordinator is responsible for all long-term care resident Minimum Data Sheets and resident MDS chart audits.  Ensures that the facility is in compliance with the Division of Aging regulations. 

Responsible for completing all long-term care MDS, admission, significant change, discharge, re-entry tracking forms quarterly and annual assessments.  Responsible for the completion of all Resident Assessment Protocol and entering all necessary information into the computer system. Maintains records of all  MDS/RAPS for a 15-month period.

 Ensures all departments involved in the Resident Assessment Instrument (RAI) process complete their MDS sections/RAPs and Care Plans in according to the time line set by the MDS Coordinator to stay within state guidelines.  Prepares and distributes time lines and calendars to all departments involved in the RAI.

 Attends the care plan meeting with all the other departments and participates in the care plan process.    

 Transmits MDS assessments successfully to the Division of Aging (minimally every thirty days, preferably weekly).

 Notifies supervisors and charge nurses of any concerns or problems in a timely fashion.

 The MDS Coordinator must conduct MDS chart audits on all admissions and/or re-admissions for accuracy and competency.

 Demonstrates flexibility in responsibilities and professional competency at all times.  Completes annual in-services and continuing education credits as required.  Keeps professional license current.  Maintains working knowledge of emergency procedures and demonstrates awareness of safety issues relevant to resident and employee safety.

 Communicates effectively with residents, visitors, co-workers, vendors, and other team membersusing professionalism.  Collaborates with Director of Nursing and other departments for problem solving and communicating resident-related issues.

 Completes other duties as assigned.
