Experience: 2 years experience as a Highland RN or 3 years RN experience from another healthcare provider; Licensed by New York State to practice as a Registered Nurse. Description: Under indirect supervision, provides and coordinates health care and professional nursing services for patients and their families. Collaborates with other health care providers as ines skills of history-taking and assessment to collect patient data Assists in identification of patient problems and collaborates/participates in plan of care based on existing standards and long-term tematically assesses patient needs using a biopsychosocial approach Identifies complex patient problems and potential patient problems Formulates ongoing plan of care using a variety of resources and incorporating long-term goals as appropriate. Coordinates interdisciplinary plan of care for patient or group of patients (i.e., acts as a case manager) Implements nursing care for complex patients. Initiates multidisciplinary involvement in care as appropriate. Recognizes trends or subtle changes in patient and/or family response to care. Coordinates modifications of the plan of care based on patient and/or family responses. Initiates and maintains mechanisms for strengthening communication at unit and/or service level. Maintains exemplary standards of documentation Monitors staff compliance with unit and/or service documentation standards. Functions effectively in charge role on an ongoing basis, coordinates assignments considering patients needs, staffs ability, unit acuity and continuity of care. Effectively manages unit, evaluates situations, sets priorities and delegates responsibilities to others. Identifies potential problem areas and independently initiates problem-solving process. Differentiates unique or complex learning needs of patient and families. Develops, implements and evaluates individualized teaching plan, adjusting approach based on learners ability.
