Job Summary:
Under general direction performs consumer assessment, care planning, case management, discharge planning, consumer care duties and program functions for adults in the Transitional Living Program.
Position Responsibilities and Duties: . Supervises and participates in maintaining a clean and safe environment. . Completes admission, discharge, and referral process for residents including new consumer orientation. . Plans and implements consumer activities, both within the facility and in the community. . Assists with planning and coordination of transportation needs of consumers. . Supervises the consumer on assigned work therapies and teach or model appropriate skills as necessary. . Schedules, as necessary, and supervises client appointments. (i.e. doctor, dental, and eye exam, etc.) . Maintains departmental files, records and/or logs according to established procedures. . Schedules, assists and assures compliance with quarterly pharmacy reviews. . Observes consumers self medicate, and possesses general knowledge of the use, dosage, and side effects of medications prescribed to residents. . Assist client in developing an individualized plan of care. Follows plan of care and documents progress daily. . Serves as a resource for the consumer's family to help answer questions, address any problems, and inform the family of changes within the scope of maintaining client confidentiality. . Coordinates the referral and discharge process to include on-going contact with referral sources or consumer regarding current progress. . Plans the preparation of food, and when necessary, prepares all meals on assigned shifts including snacks, as applicable. Monitors food inventory and completes grocery shopping weekly. . Gives on-going feedback to the Director / Clinical Supervisor regarding any non-routine events and changes in consumer behaviors that may warrant intervention. . Demonstrates the ability to assess consumer's general medical condition and refers medical problems appropriately. . Performs clerical duties including answering telephone and relaying messages, ordering supplies, inputting and retrieving information, and operating office equipment. . Utilizes safety practices in daily activities. . Develops and implements a program on continuous quality improvement and refers medical problems appropriately. . Insures compliance with regulatory agencies (i.e. Department Human Services, JCAHO and Iowa Foundation for Medical Care, etc.) . Notifies the Director / Clinical Supervisor of all staffing needs and assists with maintaining adequate staffing. . Assists in the orientation of new staff. . Performs other assigned duties.
Qualifications:
High school diploma or GED plus two years of experience in human services or equivalent combination of education and experience. Ability to assist nursing personnel with adult patients; reads, writes, and speaks fluent English. Ability to explain designated procedures and gives information; beginning skills in the use of office equipment and/or technique.
License/Registration: Iowa DOT chauffeur's license within 6 months of hire. Medication management certification is preferred. Must maintain eligibility for coverage under Mary Greeley Medical Center's auto insurance.
