Founded in 1995, Arizona MENTOR, a partner of The MENTOR Network, offers home and community-based services throughout the Grand Canyon state to adults with developmental disabilities or behavioral health needs as well as individuals with brain and spinal cord injuries or other neurological conditions. We offer a range of services - including our innovative Host Home program, as well as other residential living options, in-home support, day programs and behavioral health services.

Program Coordinators are a critical link to the community for those we serve, as well as serving as a bridge between our direct care and clinical staff to ensure the seamless delivery of services to the individuals we support.

Arizona MENTOR is seeking a Program Coordinator to provide assistance to consumers in a community-based setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight.
In addition, Program Coordinators may provide support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
Qualifications Requirements:- High school diploma or GED
- Three years related work experience
- Must be 18 years of age
- Valid driver's license in good standing
- Car registration and vehicle insurance if providing transportation for individuals receiving services
- Successful clearance of local and/or state background checks
- Supervisory experience preferred
