REM Central Lakes, a part of REM Minnesota, supports individuals with intellectual and developmental disabilities, as well as autism spectrum disorders, mental health challenges and brain injury. Through a range of home and community-based program settings, REM Central Lakes offers a range of residential services with supports tailored to the needs of each individual as well as hourly support and respite services designed to allow each individual to be as independent as possible.

Program Coordinators are a critical link to the community for those we serve, as well as serving as a bridge between our direct care and clinical staff to ensure the seamless delivery of services to the individuals we support.

REM Central Lakes, Inc. is seeking a Program Coordinator to provide assistance to people with disabilities in a community-based setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight.
In addition, Program Coordinators may provide support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
Qualifications Requirements:High school diploma or GEDAt least one or more years experience serving individuals with disabiltiesValid driver's license in good standingCar registration and vehicle insurance if providing transportation for individuals receiving servicesPrefer experience with Brain Injury.Microsoft Office experience preferred.Ability to work with and support a variety of team membersAbility to lead a team while being a team playerAbility to work with a variety of moderate medical needsSuccessful clearance of local and/or state background checksSupervisory experience preferredFounded in 1980, The MENTOR Network is a national network of local human services providers offering an array of quality, community-based services in more than 30 states to adults and children with intellectual and developmental disabilities, brain and spinal cord injuries and other catastrophic injuries and illnesses, and to youth with emotional, behavioral and medically complex challenges as well as their families.
