Immediately responsible to the Physical Therapy Supervisor or the Director of Rehabilitation Services to carry out referrals under the general direction of the referring physician. Essential Duties: Performs Physical Therapy evaluations as ordered by a physician. Based on the evaluations, develops treatment programs with appropriate treatment objectives which are mutually agreed upon by the patient. * Provided for the implementation of proper treatment programs though themselves or though supervision of a physical therapy assistant or aide. * Periodically evaluates the patient's response to treatment and alters the program as appropriate with the physician's approval. * Maintains appropriate patient records including the initial evaluation, daily charting, progress reports, discharge summary, documentation on the Patient Education Form and the Multidisciplinary Care Plan and any required communication to the referring physician. * Administers standard diagnostic or prognostic tests of muscle, nerve and joint for functional ability. * Performs therapy services in both the inpatient and outpatient populations and Home Health patients as assigned * Physical ability to perform the minimum of: floor to chest lifting of 35 lbs. 6 times per hour, generate 40 lbs. Push force and 55 lbs. Pull force, walk 500+feet and climb 4-5 flights of stairs, vision and hearing may be corrected to normal, sitting tolerance of 4-6 hours and standing tolerance of 2-4 hours. Required Licensure and/or Certifications: * * Possession of a valid license as a Registered Physical Therapist issued by the Board of Medical Examiners, State of California or eligible for license. * Current American Heart CPR certification required. Additional Information Weekend and/or Holiday Rotation may be required.
