I. JOB SUMMARY Conducts a physical therapy program to facilitate the rehabilitation of a patient who has a disability caused by disease, injury, dysfunction or other physical limitation. Patient population may include: acute care, outpatient, transitional care unit (TCU), rehab care unit (RCU), sports medicine, work hardening, home health care (FVNA) and nursing home residents. Age range of patient population served is from pediatric (6 months old) to geriatric. All the duties and standards within this job description will be performed according to the established policies, procedures and guidelines outlined in the department and Medical Center policy and procedure manuals as well as specific physical therapy laws governing the State of Ohio.
II. ESSENTIAL JOB RESPONSIBILITIES 1. Provides direct patient care: evaluates patients, establishes appropriate treatment goals, plans treatments, carries out treatment plan and documents (via: written or computerized forms) evaluations and treatments in an accurate and timely manner for patients (acute care, outpatient, transitional care unit (TCU), rehab care unit (RCU), sports medicine, work hardening, home health care and nursing home.) 2. Attends assigned meetings such as monthly department meetings, stroke team meetings and any other meetings deemed necessary and as designated by the Physical Medicine Manager or Physical Therapy Supervisor. 3. Attends job related seminars/workshops, shares information with staff (via: staff inservicing) and presents travel expense report to Physical Medicine Manager, as appropriate. 4. Supervises PTA's following established policies and procedures. 5. Attends and participates in scheduled inservices and maintains current personal inservice log for same. 6. Demonstrates a basic knowledge of and competency in clinical skills as outlined in Performance Standard 1.2 for a Physical Therapist and in compliance with the Physical Therapy Department Policy and Procedure Manual. 7. Demonstrates a basic knowledge and competency in computer - MediServe use for patient documentation and billing. 8. Complies with appropriate patient scheduling, per Policy & Procedure Manual.
III. OTHER POSITION RESPONSIBILITIES 1. Enhances positive relations with all customers, including patients, families and Medical Center staff in other departments. Enhances the practice of family-centered care and provides individual customer focus.

Demonstrates positive interpersonal relations in dealing with co-workers, supervisors, managers, physicians and other members of the Medical Center staff so as to promote efficiency and to provide quality patient care. 3. Assists Physical Therapy Supervisor and Physical Medicine Manager in the areas of budgeting, setting annual procedures, providing inservice training and education, department goals, patient satisfaction surveys. Contacts manager during "down time" to see if there are assignments to be completed. 4. Performs clerical duties: assists with scheduling, daily record keeping and billing (written and computerized); maintains files; plans and directs the receipt, storage and maintenance of supplies and equipment; prepares monthly and annual statistical reports; maintains productivity log; answers, screens and refers telephone calls and takes messages; assists visitors. 5. Assists with Physical Therapy promotional activities and program development. 6. Maintains a neat and safe work area. 7. Assists in CQI (Continuous Quality Improvement) process, develops CQI indicators and conducts CQI studies. 8. Responsible for reading and comprehending information in Physical Medicine Communication Manual weekly. 9. Assures that appropriate coverage is available for patient care during planned PTO use and provides detailed notes regarding patients to be treated. 10.Develops at least four personal goals to be accomplished annually as set at the Performance Appraisal. 11.Supervision and mentoring of volunteers and/or students, on-going, and in adherence with department specific Policy and Procedure guidelines. 12.Performs other duties as assigned. 13.The successful candidate will have to pass competencies for this position.
IV. POSITION QUALIFICATIONS Education: Bachelor's degree in related field.
Licensure/Certification: * Valid license to practice PT in the State of Ohio. * Annual CPR. * Valid driver's license and have personal transportation for performing off-site duties. * Verification of current car insurance
Experience: Preferred one year, not required.
Job Related Skills: Good interpersonal and communication skills Good working knowledge of clinical function of Physical Therapy.
