POSITION SUMMARY:
The Patient Flow Supervisor (PFS) coordinates the assignment of patient care beds within the hospital. The PFS performs initial admission screens using approved clinical criteria to assure patient admissions and bed assignments are consistent with current policy and procedure. Coordinates and expedites admissions/transfers, delays and obstacles. Works in conjunction with physician and/or staff to prioritize admissions based on medical needs. Communicates with all departments as appropriate to maximize efficiency and expediting the admission process. The PFS works in collaboration with multiple departments within the hospital and facilitates regularly scheduled bed management meetings with hospital staff to identify and communicate bed utilization issues. The PFS will identify, trend, and report barriers to patient throughput. The PFS is responsible for the operational running of the nursing departments in the absence of the nurse manager or Director.

PERFORMANCE EXPECTATIONS:
Facilitates patient throughput with an ongoing focus on quality and efficiency
Screens admissions, using approved criteria, for appropriateness of level of care orders and bed assignments
Escalates medical necessity issues, admission status issues, and bed assignment conflicts through the established chain of command
Prioritizes bed requests, coordinates bed reservations, and assigns bed reservation holds timely for all admissions and transfers in the hospital
Educates nurses, physicians, and ancillary staffs in level of care and bed placement issues
- Serves as Power User for facility and documents bed placement activities in Meditech system timely and accurately
- Through effective and professional communication, acts as a liaison between and with physicians, patient / family, hospital staff and patient care areas, and outside agencies in an effort to promote efficient patient flow.
- Communicate with the emergency department frequently to prevent a backup of admissions and activates saturation protocols to alleviate the backlog (e.g. divert to rescue according to written criteria).
- Recognizes and prevents discharge "batching" by nurses, especially near the end of shifts that delay patient discharges and unnecessary utilization of patient beds.
- Routinely rounds throughout the hospital to identify available beds on every unit.
- Keeps CNO/ACNO (as appropriate) abreast of critical situations impacting the hospital.
- Prepares a monthly written report regarding issues and concerns in the placement of patients.
- Responsively monitors facility triggers and capacity / occupancy and initiates actions to mitigate facility saturation
- Facilitates daily bed management meeting and effectively communicates resources and issues to appropriate areas of the facility
- Provides personnel to ensure appropriate staffing in accordance with budgeted matrix standards and patient care needs.
- Staffs according to ratio, patient acuity and Title 22.
- Responds to emergencies and trauma: ensures proper utilization of personnel
- Uses systematic methods to resolve problems and evaluate outcomes.
Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives as they relate to the provision of Bed Management Services
Treats customers of all ages, creeds, genders, disabilities and diagnoses with dignity and respect, without bias or prejudice.
Demonstrates knowledge of, respect for cultural and spiritual needs of others.
Maintains clinical knowledgeable around disease entities; around epidemiological issues and around circumstances that may change where a patient can be placed (e.g. ACI patients.
Tracks and trends barriers to patient flow; makes recommendations and develops action plans to improve processes and systems
Collaborates with nursing staff at all levels, interdisciplinary teams, executive leaders to discuss patient centered issues.
Adheres to established policy and procedure and standards of care; escalates issues through the established Chain of Command in a timely manner
Maintains current and accurate information in MEDITECH system
Completes Bed Management/InterQual training and serves as facility local resource for the end-user
Leads the Bed Management meetings daily at 0830 am and 1600pm. Triggers emergent Bed Management meetings according to saturation triggers.
Serves as local resource for end-user and new Bed Management/House Supervisor staff
Serves as first line liaison for problem solving for Bed Management
Actively functions as system resource by supporting parallel and go live activities for Bed Management
Adheres to corporate and facility policies and procedures
Shares enhanced knowledge and skills related to Bed Management program and technology
Reinforces staff/colleague training with hands-on Bed Management/InterQual scenarios
Accepts reassignment and additional duties, when necessary, in a flexible and positive manner.
Maintains confidentiality of patient and organizational information, including oral, written, and electronic communicating. Safeguards passwords, security codes, and other system access codes.

- Facilitates patient care through ensuring supplies from pharmacy and central supply.
- Troubleshoots unusual occurrences as appropriate to ensure optimum patient care.
- Participates and attends Nursing Division meetings. Provides input for resolution for identified problems.
- Completes all documentation on a timely basis.
- Serves as a professional role model; adheres to dress code.
- Identifies learning needs of staff to managers.
- Counsels employees in conjunction with the department Director/manager in accordance with hospital policy, employment laws and appropriate contract.
- Demonstrates good telephone courtesy. Conveys caring by both verbal and non-verbal communication, eye contact, and appropriate use of touch.
- Fosters a culture of safety and a "no blame" environment for the organization.
- Follows established guidelines for reporting a significant medical error or unanticipated outcome in the patients care which result in patient harm.
- Attends in-service presentations, and completes mandatory education including, but not limited to, InterQual guidelines, infection control, patient safety, quality improvements, MSDS and OSHA standards.
- Actively supports improved compliance with external patient safety initiatives: HCA, TJC, Leapfrog, and NQF.
WORK ENVIRONMENT: This position operates within the confines of the hospital building and general office environment. Hours and shifts may vary to include holidays, weekends and evenings.
REPORTS TO: ACNO
DEGREE OF SUPERVISION: Minimal, must be able to work independently
EQUIPMENT USED: Computer, telephone, calculator, fax, copy machine and pager.
Qualifications:
POSITION QUALIFICATIONS:
RN with current state licensure, BSN required, MSN preferred
5 years nursing experience in acute care setting, prefer multi unit experience.
Manager experience in an acute hospital setting preferred.
Ability to establish and maintain collaborative and effective working relationships
Ability to communicate effectively in oral, written and electronic formats
Demonstrated analytical and critical thinking abilities with pro-active decision-making and negotiation skills
Demonstrates abilities as a mentor/coach by assessing needs, planning, implementing and evaluating learning and performance of team members as assigned.

Ability to manage multiple tasks simultaneously
Competent in the application of medical necessity criteria (i.e., InterQual)
Must be licensed to be able to take an order from a physicianMust be proficient in MEDITECH usage and computer literate to include knowledge in Microsoft Word, Excel and T-system.
Knowledge of Title 22, TJC, Nursing BRN standards and hospital policies.
Competent in assessment and revision of organizational policies.
