Responsibilities Coordinator - Patient Care (R.N.) Location: One Ford Place Full-time, days and weekends On-call coverage required
- Assesses and identifies patient needs in accordance with the hospice concept of care. - Coordinates with interdisciplinary team to provide complete patient/family care and referral. - Maintains the continuity of care by ensuring that patient needs are met.
