Responsibilities Patient Care Coordinator Full time, Days Hospice Hours: 8:30am to 5:00pm
Responsibilities:
Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Maintains the continuity of care by ensuring that patient needs are met.
Experience and Education:
Requires graduation from an accredited school of nursing, current Michigan licensure as an RN, understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management. Requires some on-call. Requires a valid drivers license in the State of Michigan and an acceptable driving record. Background in oncology nursing or previous hospice experience preferred.
