Outreach Coordinator
The Charles H. Best Diabetes Centre for Children and Youth, a charitable organization operating in Durham Region, is hosting the new CE LHIN Regional Diabetes Coordination Centre or Diabetes RCC. The Diabetes RCC is part of the Ontario Diabetes Strategy. We are currently accepting resumes for the following position: Full-time Outreach Coordinator Key Responsibilities: Build awareness of diabetes services and participates in the dissemination of diabetes standards of practice across the region. Works collaboratively with other groups and organizations in the region (primary care physicians, Family Health Teams, Nurse-Practitioner Led Clinics, LHINs, diabetes education programs and other agencies) to assist with diabetes service coordination. Works as part of the Regional Coordination Centre (RCC) team to achieve the goals and objectives of the Diabetes RCC and the Ontario Diabetes Strategy (ODS). Assists in the development and implementation of short and long-term strategies to enhance diabetes service provision. Required Qualifications: Bachelors degree in a health related discipline or equivalent Current registration in good standing with applicable Health Care Regulated Professional College Masters Degree in health care related field or equivalent preferred. Certified Diabetes Educator preferred Minimum 3 to 5 years experience within the health care sector preferably at the regional and/or provincial level. Demonstrated experience in integrating initiatives across multiple sectors. In-depth understanding of the provincial health-care system, its stakeholders (primary care physicians, Family Health Teams, LHIN and diabetes education programs, community agencies), programs and issues. Experience with outreach initiatives and program development Experience in diabetes and chronic disease management preferred Experience in capacity building activities and initiatives Possesses highly developed analytical, critical thinking, and organizational skills. Demonstrates excellence in interpersonal, communication, mentoring, coaching, and team building skills that facilitate positive interaction among all members of the Diabetes RCC team, internal and external partners Demonstrates superior understanding of systems and system analysis techniques, value analysis, continuous improvement principles and tools and customer orientation. Flexibility in working hours and available to travel Experience in effective collaborative relationships with external organizations, including academic, private and public sector organizations Ability to express complex ideas clearly and effectively to a non-technical audience. Valid Driver's license is required Demonstrates excellence in communication, both written and verbal Analysis / problem assessment Innovative and willing to learn Demonstrates individual leadership skills This full-time position is funded through the Diabetes Regional Coordination Centre.
