Responsibilities Applies the appropriate diagnostic and procedural codes to individual patient health information for data retrieval, analysis, and claims processing for outpatient, clinical diagnostic laboratory services, clinical diagnostic medical imaging services, non-implantable prosthetic and orthotic devices, physical, occupational and speech therapy, diagnostic screening mammography, and all other ancillary procedures. Abstracts pertinent information from patient medical records. Assigns ICD-9-CM, CPT/ HCPCS level II codes, creating Ambulatory Payment Classification (APCs) assignments. Queries physicians when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or unclear for coding purposes. Identifies discrepancies between coded data and supporting documentation. Consults reference material to facilitate code assignments. Keeps abreast of coding guidelines and reimbursement reporting requirements. Follows-up on charts ensuring that National and Local Coverage Determinations (NCD/LCD) for medical necessity are met and makes any necessary corrections as applicable. Provides feedback for corrections. Performs audits of records; makes any necessary corrections as applicable and provides feedback as appropriate. Runs a weekly list of outstanding charts and submits it to the supervisor for immediate follow-up on old accounts. Brings identified concerns to supervisor for resolution.
Education Requirements High School Diploma, Completion of coding program, Medical Terminology classes. Associate degree in Health Information Technology preferred. Completion of course work in anatomy and physiology, pharmacology, pathophysiology preferred. Certified Coding Associate (CCA) required with in one year of hire.
Minimum Experience, Knowledge, Skill, and Ability Requirements Outpatient Coding Specialist 1: Six (6) months experience in health information management/coding in a healthcare organization, with knowledge of ICD-9-CM, and CPT-4, coding conventions and guidelines or no experience needed with CCA certification and completed coursework in anatomy and physiology, pharmacology, or pathophysiology. Detail oriented, possess good problem solving skills, and have legible handwriting. Skilled in using HIM software. Good communication skills. Analytical and problem-solving skills.
