Each of our facilities (Toronto General Hospital, Princess Margaret Hospital and Toronto Western Hospital) makes a unique contribution to the whole. Together, we create something far greater than the sum of our parts. The Nurse Practitioner (NP), having a specialty-based focus, functions in an expanded nursing role demonstrating a high level of autonomy and expert skill to formulate clinical decisions. The NP practices according to the philosophy and standards of UHN and the Division of Nephrology and integrates knowledge and skill from nursing and medicine in order to assess, diagnose, treat and evaluate elderly individuals who have renal failure. Care is extended to individuals aged 65 years and older with renal failure and their families, treated on an out-patient basis or admitted to University Health Network (UHN) and affiliated clinical areas, including Toronto Rehab Institute (TRI) and/or the Bickle Centre. The role is a balance of independent practice and collaboration with the Nephrology and Geriatric interdisciplinary health care teams and involves assessment and care of patients referred to and participating in the dialysis rehabilitation program at UHN as well as the geriatric renal management out patient clinic. QUALIFICATIONS
* Completion of a masters degree program in Nursing * Acute Care Nurse Practitioner Certificate completed, or in progress * Registered Nurse (EC) certification, or in progress * Three to five years clinical practice in Nephrology nursing in an acute treatment center * Nephrology certification (CAN) required * Gerontological certification preferred * Experience in nursing research methodology * Eligible for cross-appointment to an academic institution * Membership in the Registered Nurses Association of Ontario (RNAO) and CANNT * Excellent verbal and written communication skills * Effective interpersonal and customer services skills * Excellent organization and time management skills * Excellent decision making, problem recognition and problem solving skills * Excellent negotiation and conflict resolution skills * Program planning and evaluation methodology skills * Commitment to collaborative practice * Ability to work effectively independently and under stressful conditions * Ability to work in a self directed manner * Ability to manage and prioritize multiple demands * Ability to utilize information technology
