Horizon Healthcare Associates, a premier multi-specialty practice in Chicago’s south suburbs, is seeking an experienced Nurse Practitioner. In this position, you will be responsible for providing care to the Group’s dialysis patients and also for conducting office based clinics for primary care patients. Interested candidates should possess substantial clinical experience with Internal Medicine patients and be willing to work with the Group’s 450 dialysis patients. The remaining 50% of your time will be spent with Primary Care patients.

Responsibilities include providing and maintaining a quality, customer-focused, nursing and medically appropriate approach to patient assessment, care plans development and implementation, and follow-up. The group has recently implemented an electronic medical record.

Requirements include:

- Master of Science Degree in Nursing
- Completion of Nurse Practitioner program and Certification as a Nurse Practitioner
- Current license to practice as a Registered Nurse and as an Advanced Practice Nurse
- Current Certificate to Prescribe
- Minimum of 5-10 years’ experience as a Registered Nurse or Nurse Practitioner
- Prefer 1+ years of experience as a Nurse Practitioner in a Clinic setting
- BLS certification required
