Effectively manage chronic disease including clinical management, relationship building, patient education, activation and engagement.
Utilizing advanced health assessment, knowledge, and decision making skills, the Nurse Practitioner or Physician Assistant works independently and in collaboration with physicians and other health care professionals.
Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.
Education/Experience: PA: Bachelor's degree in a health/science-related field. NP: Masters degree in nursing.
Licensure/Credentials 1. Current licensure as a physician assistant or nurse practitioner in appropriate state. (NP) Current licensure as a registered nurse in appropriate state. 2. Current certification in CPR. 3. National certification. 4. Successful completion of SMDC credentialing process prior to practice
