Reports to the Director/Chronic Care Services - Responsible for program oversight of Diabetes, Wound, and Hyperbaric Centers. Provides leadership in the coordination of departmental activities and the resolution of operational and patient care issues. Integrates decisions of committees into the operation of the department. Incorporates a high quality, cost effective program to provide optimal health care to patients.Coordinates and integrates services areas within the hospital environment and community for pediatric, adolescent, adult and geriatric age groups. Actively attends management committee meetings. FUNCTIONAL RESPONSIBILITIES: * Coordinates and integrates diabetes and wound care management within the hospital environment and community for pediatric, adolescent, adult, and geriatric age groups. Coordination of patient care. * Ability to function successfully in an extremely challenging environment. * Establishes a course of action for self and others to accomplish specific goals; planning proper assignments, and effective and efficient allocation of resources. * Actively attends Management Committee meetings. QUALIFICATIONS: * Bachelor's Degree in Nursing or associated field is required. * Current licensure by Michigan State Board of Nursing is required. * Certification in Diabetes Education is desired (or must be obtained within 3 years) * Current knowledge and clinical skills in area of diabetes and would care is required. * Familiarity with wound and ostomy assessment and basic principals of management and patient teaching. * Annual BCLS certification is required. * Previous management experience. * Computer skills in word processing and spreadsheet software. Analytical skills necessary to collect, tabulate, and analyze data to determine and implement effective action and solutions. * Demonstrates leadership ability, communications, and interpersonal skills necessary to interact effectively with physicians, management, staff, external agencies/customers and patients/families. * Analyzes costs and work schedules and set priorities. * Responsible for departmental personnel matters pertaining to the employment, training, termination and grievance of employees. * Physically capable of standing, walking, stooping, bending and lifting up to 25 pounds.
