Job Summary: Responsible for providing social services and addiction services to the patient and family by assessing, planning, implementing, and documenting patient care and discharge planning. Other duties as assigned.
Experience: A minimum of one year experience, preferably in a similar setting, required.
Education: Bachelor's degree in social work or counseling from an accredited college or university required.
Training: Substance Abuse Counselor In Training (SAC-IT) or obtained within one year of hire.
Special Skills: Mandt Training within 90-days of hire.
Licensure: Valid Wisconsin driver's license. Current licensure as a Certified Social Worker or Counselor. Certified Substance Abuse Counselor (C-SAC) or obtained within one year of hire.
