Visiting Nurse & Health Services is seeking a qualified candidate to be responsible for tracking and maintaining ongoing Face to Face documentation in compliance with CMS guidelines.
Candidate will also be responsible for:
1. Questions/Issues regarding Face to Face compliance and billing regulations will be
directed to Vice-President of PQI.
2. Maintains daily log and status of received Face to Face documentation. Review documents
for accuracy and processes completed documents to HIM.
3. Develop, implement and maintain tracking spreadsheet for all Face to Face documentation
and pending list.
4. Telephone calls to follow-up with physicians, SNF or other referral sources regarding
the completion of Face to Face requirements.
5. Works with family and patients for follow-up physician appointments.
6. Input Peco’s/NPI number of new physician.
7. Utilizes Agency generated reports to support Agency tracking spreadsheet.
a) Notifies and works closely with Referral Center/Clinical Managers regarding
patients who have been re-hospitalized or those patients having difficulty
obtaining physician visits, or pending Face to Face in compliance with Face to
Face regulations.
b) Maintains, implements and distributes weekly reports to Clinical Managers.
c) Provides back-up to Intake.
8. Covers referral center as needed. Responsible for identifying and obtaining all necessary
information for referrals.
Requirements:
Qualified candidate to have LPN license or medical knowledge and experience. Candidate must possess excellent customer service skills, organizational skills, communication skills, must be detail oriented and able to work in a fast -paced environment while prioritizing tasks.
