Celebrate Caringat Bay Tree Care and Rehabilitation Center, an affiliated center of SunBridge Healthcare and a leader in long term and rehabilitative healthcare. We currently have an opportunity for an MDS Coordinator, RN, at our 120-bed center located in Palm Harbor, FL. This position is Mon-Friday. We offer excellent company training and support for MDS.
Responsibilities include: · Complete nursing assigned MDS items, RAPS, PPS and Care Plans. · Direct the facility interdisciplinary RAI process while following company guidelines for completion, computer encoding and electronic State transmission of the MDS. · Participate in the resident Care Plan Meetings.
Job Requirements : · Must be a Registered Nurse or Licensed Practical Nurse in good standing and currently licensed by the State; · Previous experience as an MDS Coordinator and knowledge of RUGS optimization. · Previous experience in completing MDS, RAI process and Care Plans is required. · Previous experience in a hospital, long-term facility, or other healthcare related facility. · Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care. · Positive attitude toward the elderly.
