Provena Cor Mariae, Rockford, ILis a 162 bed, Medicare and Medicaid Certified nursing facility that offers rehabilitation, LTC, sheltered and an independent living setting in a resident choice environment.
LPN
JOB SUMMARY:Responsible for the provision and supervision of nursing care for Cor Mariae residents pursuant to resident care plans and physician orders. With aide of registered nurse, supervises nursing assistants and health service aides. Manages his/her assigned unit in compliance with nursing standards, federal/state nursing regulations and the policies and procedures of Provena Cor Mariae. Responsibilities include defining, planning, organizing, directing, managing, implementing, and evaluating nursing services provided to Cor Mariae residents.
QUALIFICATIONS:
1. Current license as LPN in the state of Illinois in good standing with the
Department of Professional Regulations.
2. Able to work with limited supervision and to provide supervision when
needed.
certification in CPR.
JOB ANALYSIS (FUNCTIONS):
1. Performs and documents all resident care as outlines in the resident's
care plan.
2. Provides and documents delivery of care, observations, medications and
treatment promptly and accurately.
3. Accompanies physician on rounds.
4. Responds appropriately to emergencies.
5. Supervises nursing assistants in resident care.
6. Assists in determining proper nutrition.
7. Maintain confidentiality and privacy of all residents.
8. Practices established infection control policies.
9. Provides accurate and proper shift reporting.
10. Provides for security measures in the absence of nursing/administration
management including proper door monitoring, locking etc.
GENERAL REQUIREMENTS:
1. Adhere to federal and state regulations governing long term care.
2. Maintain a neat, well groomed appearance and adhere to the dress code of
the position.
3. Comply with infection control guidelines and follow universal precautions
and isolation procedures.
4. Must be knowledgeable of Provena Cor Mariae Center's fire and safety
procedures.
5. Project a positive image when dealing with residents, coworkers, families
and the public.
6. Attend all mandatory in-services.
7. Work cooperatively with co-workers and other departments.
8. Responsible for knowing and doing what is right and reporting any legal,
ethical or safety concerns to the appropriate party.
