Provena St. Anne Center, located in Rockford, Illinois is a 179-bed, Medicare certified bed facility offering post-acute, rehabilitation and LTC nursing services in a resident choice environment.
Licensed Practical or Registered Nurse
JOB SUMMARY:
Responsible for the provision and supervision of nursing care for St. Anne residents pursuant to resident care plans and physician orders. With aide of registered nurse, supervises nursing assistants and health service aides. Manages his/her assigned unit in compliance with nursing standards, federal/state nursing regulations and the policies and procedures of Provena St. Anne. Responsibilities include defining, planning, organizing, directing, managing, implementing, and evaluating nursing services provided to St. Anne residents.
QUALIFICATIONS:
Current license as LPN or RN in the state of Illinois in good standing with the
Department of Professional Regulations.
Working knowledge of nursing regulations governing long term care.
Able to work with limited supervision and to provide supervision when
needed.
Certification in CPR.
JOB ANALYSIS (FUNCTIONS):
Performs and documents all resident care as outlines in the resident's care plan, provides and documents delivery of care, observations, medications and treatment promptly and accurately, accompanies physician on rounds, responds appropriately to emergencies, supervises nursing assistants in resident care, assists in determining proper nutrition, maintain confidentiality and privacy of all residents, protects residents and staff from accident/injury by following safety and fire procedures, practices established infection control policies, provides accurate and proper shift reporting, keeps the nursing station, med room and med cart neat and orderly, uses appropriate abbreviations as outlined in procedure book, provides for security measures in the absence of nursing/administration
management including proper door monitoring, locking etc, monitor and meet resident needs consistent with quality nursing procedures, such as bathing, transferring, eating, walking, etc., other duties as assigned by the DON, ADON, or the nurse leader.
SUPERVISORYFUNCTIONS:
With input from registered nurse, assigns and directs nursing assistants and health service aides in the provision of resident care. Such assignment and direction includes, but is not limited, to the following:
Assigning the aforementioned employees to care for particular residents, assigning and directing employees in the performance of their job functions, monitoring employees to ensure that proper care and service is provided to St. Anne residents and responding to problems in resident care accordingly, assigning employees to in-services as scheduled.
Ensures that individuals under his/her supervision conform their behavior and job performance to St. Anne policies and procedures, provides direct input in the disciplinary process including the issuing of verbal warnings, written warnings, suspensions and terminations and is involved in both disciplinary and termination meetings, evaluates resident care and personnel emergencies and responds accordingly, involved in the recruitment and hiring of individuals under his/her supervision, recommends the transfer of employees to other units, with registered nurse input, determines appropriate staffing levels for the unit and adjusts those ratios as necessary. This may include staffing increases or reductions and reassignments.
