Heartland Hospice, a subsidiary of HCR ManorCare, is currently searching for PRN LPN's to support our hospice patients located in Dane County. These positions are needed on an as needed basis and we are open to availability to include days, evenings, NOC's or weekends. Continuous Care/Crisis care is our primary need.
This position is responsible for the delivery of skilled nursing care and supportive services within the scope of the nurse's education and experience. Key responsibilities involve coordinating care delivery to ensure that clients' needs are met in accordance with professional standards of practice through physician orders, Heartland policies and procedures, and federal, state and local guidelines.
