Home Care Case Manager Department: Home Care Schedule: Full Time Shift: Day Hours: Job Details: - RN - 1-3 years
Under the direction of the Home Care Manger, performs primary nursing care functions in conformance with professional standards of home care nursing practice under the plan of care approved by the primary physician. The Home Care nurse coordinates all services provided for each assigned patient and family. Regular Local Travel required- Baltimore County West, and some Baltimore City. QUALIFICATIONS: Currently licensed as registered nurse in State of Maryland. Previous medical and surgical nursing experience. Home Care experience preferred. Must have valid drivers' license and own transportation. Must be able to turn and position a patient up to 200 lb. Must be able to transfer a patient up to 200 lb. independently or with assistance available in the home. Must be able to climb stairs. Must be able to tolerate household allergens, e.g., pets, cigarettes, perfumes, etc.
