Health Care Coordinator (RN) Department: Adult Day Center Schedule: PRN Shift: 1st Hours: Monday - Friday 4 to 8 hour shifts, times as needed. Job Details: * Associate's Degree * RN Required * Position Summary: Performs the substitute function of the Healthcare Coordinator in assessing, planning, implementing and reporting all assigned patient care in the absence of the HCC. Is responsible for all patient care he/she gives during a shift (4-8 hours M-F as determined by Program director). Is responsible for adhering to Adult Day Care and Day Health Services Standards and DFS Standards of Nursing Practice, for managing supplies and equipment, and for promoting teamwork with physicians, families, and staff of Day/Day Health program. CPR -AED/First Aide Certified. Qualifications: Graduate from an accredited school of nursing. Holds a current North Carolina License Registered Nurse. Familiar with the standards of Adult Day Care and Day Health Services. Follow written or verbal instruction effectively; calculate and administer medications according to Granville Health System/Harold Sherman Adult Day Center and state standards policies and procedures; monitor effectively the care of the patient and related documentation to assure adequate care is administered. Provided hands on educational conferences, completes educational inservices and updates and maintains professional knowledge and skills related to areas of practice. Ability to relate to people, has patience, positive mental attitude, ability to listen, and sense of responsibility to the program.
