•Lead Nurse Practitioner

Other Major Relationships:
•Will maintain good working relationships and actively collaborate with the Pharmacist, Pharmacy Manager, and retail partner Store Manager plus support staff. Actively work with clinic support staff both located in the market and overall company. Maintain a good working relationship with collaborating physician(s).

Position Summary:

The Cover Provider (TCNP or TCPA) believes in excellence and is passionate about providing outstanding patient care. The Cover Provider will be responsible for providing episodic care, screening and education for patients 18 months or older. The Cover Provider will be required to be available in order to be scheduled any day of the week to staff a market of clinics for coverage for paid time off, leave of absence and unanticipated staffing gaps. Schedules vary from week to week. Schedules will be posted 60 days in advance
•Full-time Status: 30 to 40 hours per week.
• Weekend requirement is up to 3 full weekends (Saturday and Sunday)
•Regular Part-time Status: 20 to 29 hours per week.
• Cover Regular Part Time may work as little as zero hours in pay week and may work up to 40 hours in next pay week.
• Weekend requirement is up to 3 full weekends (Saturday and Sunday)

Position Essential Functions:

•Provide episodic care, screening and educational support to our Take Care Health patients.
•Extend warm, compassionate care to all with a respect for the diversity of all patients.
•Committed to excellence, best practices, and superior customer service with every patient.
•Participate in a collegial fashion with all Take Care Health colleagues.
•Participates in ongoing quality assurance audits to include peer review, medication administration review and education as well as patient concerns/complaints review.
•Support and follow the most current evidence-based guidelines of clinical practice while providing high quality patient care.
•Provide diagnostic screenings such as cholesterol and blood glucose as outlined in the TCHS evidence-based guidelines.
•Provide common vaccinations such as TB, MMR and Influenza as outlined in the TCHS evidence-based guidelines.
•Fully embrace and utilize the TCHS latest technology to provide patients with access to their medical records and fast, accurate prescriptions.
•Participate in ongoing nursing education.
•Maintain current state nurse practitioner/physician’s assistant licensure.
•Keep informed of healthcare public policy.
•Continuously communicate with TCHS leadership to enhance our patient centric experiences at our Take Care Health clinics.
•Complete training on TCHS’s electronic medical record and information systems; utilize the systems to the fullest capacity to provide superior patient care documentation.
•Performs other duties as deemed necessary by the Lead Nurse Practitioner, Regional Nurse Practitioner and Chief Nurse Practitioner Officer.


Position Qualifications:

•Licensure requirements:
oValid Advanced Practice State License issued by the State Board of Nursing.
oValid accreditation from the AANP or ANCC as a Family Nurse Practitioner.
oValid Prescriptive Authority (per state practice requirements).
oMeet and maintain all legal requirements per state and national statutes to practice within state (for your market) as a family nurse practitioner.
oCPR Certification.

•Minimum of eight (8) months of family nurse practitioner clinical experience.
•Recent experience with pediatric patients (18 months and older).
•Meet and maintain all legal requirements per state and national statutes.
•Team oriented and passionate.
•Enthusiastic in the delivery of compassionate, quality patient centric care.
•Excellent communication skills that will enable an informed patient experience; and, the promotion of our quality of care within the clinic, within our retail partner and within the community.
•Basic business skills that will foster a pleasant patient experience from check-in through our Kiosk concluding with check-out (cash collection or insurance information gathering) through our electronic medical record “EMR”.
•Ability to complete training and utilize to the fullest capacity the TCHS electronic medical record and information systems.
•Strong organizational skills with the ability to multi-task.
•Pleasant interpersonal demeanor and ability to work with all levels of colleagues and leadership.

While performing the duties of the job, the colleague will be required to meet the demands of the following requirements: sit, stand, talk and hear. In addition, the colleague must have the ability to:
•Work a 12 hour shift
•Sit up to 6-8 hours/day
•Stand up to 4-6 hours/day
•Bend to reach supplies/materials occasionally
•Reach with hands and arms in excess of 20 inches frequently throughout the day
•Reach to elevated supplies/materials, occasionally to heights of 72-75 inches, and regularly to heights of 55-65 inches
•Use a step-stool, as necessary, to reach elevated materials
•Lift materials up to 10 pounds frequently, up to 20 pounds occasionally
•Grasp patient diagnosis tools
•Key information into a computer workstation
