Overview This is the opportunity for you if you: • Are committed to serving a very diverse clinical population including obstetrics, prenatal, patients with chronic diseases as well as the elderly • Would like an opportunity to work in multidisciplinary teams, consult with primary care providers and behavioral health specialists on specific nutritional therapeutics • Have experience with cultural diversity, research and public policy • Enjoy being a part of an outstanding team of professionals As a Registered Dietician for Community Health Center, Inc. you will: • Demonstrate your expertise and commitment to nutritional counseling and education to individual clients and groups • Provide direct care to patients in multiple sites of Connecticut's largest community health center • Support and champion community education as well as direct clinical intervention • Provide community outreach and nutritional education to other social/community service agencies • Assess client's nutritional needs accordingly • Develop and institute a plan of care in collaboration with the client and health care provider • Present in-service education for agency staff • Communicate effectively with clients, providers and staff • Document nutritional assessment and plan of care accurately • Report granting information as needed
Qualifications • Bachelors of science degree in nutrition and maintenance of professional credentialing and licensure • Minimum two years clinical experience preferred (will consider newly registered dietician with exceptional academic and internship background.) • Commitment to community health populations • Bilingual/bicultural abilities preferred
