Diabetes Education Coordinator Department: Care Management Schedule: Full Time Shift: Hours: Primarily Days Job Details:
The Diabetes Education Coordinator is a diabetes educator (DE) and an advanced practitioner who is an expert clinician and advocate for diabetic patients. The DE provides direct patient care, including assessment, diagnosis, planning, and treatment of health problems, health promotion and preventive care to the patient with diabetes. In addition to direct practice, the DE provides staff/patient/family education, evaluation of patient care outcomes, conducts clinical research and data analysis, provides professional consultation and promotes interdisciplinary collaboration.
Masters degree in Nursing required, preferably as a Nurse Practitioner or a Clinical Nurse Specialist. Movement toward certification in diabetes education required. Three years experience in specialty area and diabetes education preferred. Previous experience in application of research strategies and program management also preferred.
