Duties: Essential Job Functions and Responsibilities:
The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. They are not intended to be an exhaustive list of all duties, responsibilities and skills required of personnel so classified.
* Has appropriate knowledge and skills to coordinate, facilitate, and organize a referral process for the home heath programs
* Has appropriate nursing knowledge and skills to function as an LVN/LPN
* Has an understanding of the Nurse Practitioner Act and the state and federal regulations of home health care
* Prepares reports accurately and timely
* Communicates effectively both oral and written
* Demonstrates ability to work cooperatively
* Plans and organizes work effectively and efficiently
* Demonstrates effective problem solving skills
* Demonstrates dependability
* Utilizes resources effectively and efficiently
* Participates in professional development and training activities
Qualifications: Qualifications:
* Graduate of an accredited school of vocational nursing
* Licensed as a vocational nurse in the state where practicing
* At least one year of clinical vocational nursing experience, preferably in rehabilitation or medical/surgical nursing
* Experience in the provision of in-home services to the elderly and/or disabled
* Home health experience strongly preferred
* Ability to make home visits and provide nursing care in the home environment
* Ability to complete necessary documentation appropriately, legibly, and timely
* Skilled in making effective verbal and written communications with subordinates, coworkers, consumer, and referral agencies
* Skilled at conflict resolution
* Ability to conduct training sessions with home care aides in group settings or individually
* Must have a vehicle, current driver's license and appropriate automobile insurance
* Knowledge and experience in areas the areas of home health care, hospice, or related health programs
* Ability to work effectively with health care providers, commercial vendors, government agencies, community groups, and other organizations as necessary
* Skill in dealing effectively with people at all levels
* Skill in analyzing and solving problems
* Ability to prioritize and concurrently manage multiple tasks
* Ability to identify hazards, make reports and/or implement corrective action
* Ability to respond in a calm, professional demeanor
* Ability to work in stressful situations or environment
