Continuous Care Nurse PRN (RN & LVN)
Continuous Care Nurses needed to provide medical care and emotional support to terminally ill patients. Duties and responsibilities include: -Perform all basic nursing skills within an LVN's or RN's scope of practice Actively participate in the coordination of patient's care, in accordance with current professional standards and practice -Conduct patient visits as scheduled by the Plan of Care -Implement all direct care as assigned by the Patient Care Manager Contribute to patient and family education -Maintains complete, accurate, and up-to-date medical documentation; Submits documentation in a timely manner, per organizational requirements -Participates in QAPI program and hospice in-service training We currently serve Dallas, Collin, and Denton counties. - Medical, Dental & Vision - Life Insurance & AD&D - Long Term & Short Term Disability - Flexible Spending Account - 401k Plan - PTO - In-Service & Educational Opportunities Faith Hospice, was founded in 2004 and in 2008 became a member of Presbyterian Communities and Services changing its name to Faith Presbyterian Hospice.
