CLINICAL TRANSFORMATION PHARM Department: CLINICAL TRANSFORMATION Schedule: Part-time Shift: Dayshift Hours: Weekdays,7a-5p Job Details: * The clinical transformation specialist (Pharmacy) will be responsible for working with and reporting to the Chief Medical Information Officer (CMIO), for the consolidation of physician order sets and the development of standardized institutional order sets for specific diagnoses. As a member of the Clinical Transformation "team", the clinical transformation specialist (Pharmacy) will play an integral role in the development of evidence-based care standards and subsequent translation of these standards into a computerized provider order entry (CPOE) library by working in conjunction with the CMIO, the medical staff of RMH, and all ancillary services involved patient care delivery at the direction of the medical staff. This Pharmacist / Liaison position will simultaneously provide for input from and education to the medical staff, nursing staff and all other ancillary services involved in patient care delivery at the direction of the medical staff. The ability to develop a strong working relationship with physicians, Health Information Management (HIM), pharmacy, nursing and information services (IS) will be essential for this position. The clinical transformation specialist (Pharmacy) must understand and use a "systems" based/results oriented project approach and hospital experience (managerial level preferred). In addition must have a working knowledge of the RMH physician order process, a working knowledge of the multiple interfaces between physicians and ancillary services, and very strong communication skills. The clinical transformation specialist (Pharmacy) will be expected to quickly learn about clinical transformation, evidence-based care and certain parts of IT clinical applications in order to quickly become a fully functioning member of the Clinical Transformation Team. Minimum Education:Bachelor's degree in Pharmacy required. Residency or Pharm D preferred. Minimum Work Experience:Inpatient hospital experience with extensive order entry and clinical experience required. Must have basic knowledge of Meditech and documented RMH competency with 30 days of hire. Must have advanced Meditech skill competency documented within 30 days of being hired. Experience in creating a successful collaborative work environment using effective interpersonal and communication skills is required. Demonstrated project management skills are required. Required Licenses:Current Virginia License to practice pharmacy in the State of Virginia. Physical Requirements/Environmental Conditions:Performance of administrative duties usually will be accomplished during a standard daylight work week; however, the employee may occasionally work other shifts or hours to fulfill responsibilities and maintain visible support for the RMH clinical transformation process.
