The Clinical Pharmacist for Medicare Preferred will become part of a multi-disciplinary case management team contributing to improving quality of care through care coordination, medication management programs, consultative services, and other quality initiatives. The pharmacist is responsible for providing medication consultations directly with targeted members, writing and maintaining education materials both for providers and members, as well as designing medication adherence programs for the network. The pharmacist will support the case management team as subject expert on medication management as well as benefit and formulary information. Qualifications:
EDUCATION:(Minimum educations & certifications required) Pharm D from NABP accredited institution. Current registration in good standing with the Board of Pharmacy.
EXPERIENCE: (Years of experience) Minimum 2-5 years experience in a clinical or health plan setting. Geriatric specific and program development experience a plus. Expert knowledge of the principles and practices of pharmaceutical science, a working knowledge of Federal and State Controlled substance and pharmacy laws, rules and regulations.
SKILL REQUIREMENTS:(Include interpersonal skills) Strong communication skills including ability to listen effectively, excellent written communication abilities and proficiency in communicating across disciplines internally and externally (ie. with MDs, colleagues, members, and caregivers).
Must be self-motivated, capable of working independently while organizing, facilitating and managing several projects simultaneously. Must be flexible and able to constantly review and reset priorities.
Strong computer PC skills including MS Office (Word, Excel, PowerPoint & Access) and ability to learn/use new database and reporting applications.
Requires analytical ability to independently research/ summarize data on drug utilization and spend.
WORKING CONDITIONS AND ADDITIONAL REQUIREMENTS(include special requirements, e.g., lifting, travel, overtime) Work is performed at the office. Must have the ability to operate a computer and access and utilize file cabinets, bookcases, machines and copiers.
