Description The Clinical Coordinator is an advanced clinical pharmacy practitioner. The Coordinator is responsible for providing and coordinating clinical pharmacy services to meet the needs of patients and support the mission of the Department of Pharmacy and the Health System. The Coordinator evaluates both individual patient and care plan drug therapy and resolves drug related problems and issues, including the individualization of patient specific therapies to optimize pharmaceutical care. He/She serves on one or more Medical Staff, hospital, or departmental committees or subcommittees. The Clinical Coordinator serves as a role model for Clinical Pharmacists, pharmacy students, and residents in the integration of clinical and dispensing services. He/She plans and participates in staff development conferences for the professional and technical pharmacy staff and educates other health care professionals to support optimal patient care. Minimum Job Qualifications Minimum Education: 4 year degree. Preferred Education: Advanced degree. Minimum years in field previous to employment: 2. Completion of an accredited residency or at least two years of experience working in a specialty practice area required. Completion of specialty residency preferred.
