We are seeking experienced nurses who would like a Monday through Friday daytime schedule in a corporate environment and a competitive salary/benefits package, including paid holidays and paid time off. Due to our continued growth, we are currently seeking RNs & LPNs to work in our Clinical Communication Centers in Maryland and Michigan. This is an office-based position which requires a well-rounded clinical background, excellent communication and teaching skills, a consistent work history, strong computer skills, and a desire to make a difference in the lives of others. This is a great opportunity to use your solid clinical knowledge to help individuals with chronic diseases (diabetes, cardiovascular, etc.) learn how to manage their disease process and maintain a healthier lifestyle. Responsibilities: Complete telephonic health risk assessments or screenings that facilitate patient stratification and the identification of appropriate evidence-based interventions designed to improve patient health outcomes; Based upon ongoing reviews of accumulated patient information, implement interventions and services according to pre-established protocols and care plan guidelines designed to achieve chronic care program patient goals; Communicate professionally with patients, providers, and caregivers to obtain or provide information, including completing telephonic health counseling and education calls; Provide disease-specific education in accordance with corporate policies, procedures and program content; Advance knowledge base and communication skills through training, mentoring and observation; Maintain compliance with MedAssurant policies and procedures and client organization contractual agreements; Adhere to all confidentially and HIPAA requirements as outlined within MedAssurant policies and procedures and your Employee Agreement in all ways and at all times with respect to any aspect of the data handled or services rendered in the scope of work; and Fulfill those responsibilities and/or duties that may be reasonably provided by MedAssurant for the purpose of achieving operational and financial success of the Company. Requirements Registered Nurses (RN) & Licensed Practical Nurses (LPN/LVN) with active license; Familiarity with chronic care conditions offered by MedAssurant to client organization; Demonstrated phone communication experience with simultaneous ability to document findings in a computer and the ability to manage and meet deadlines; Highly computer literate and detail oriented; Professionally skilled in personal presentation, verbal, and written communication; and Meticulous organizational skills. 7. Hours will be 11:30 a.m. - 8 p.m. Monday - Friday.
