Job Posting Title Clinic Care Coordinator City and State Brainerd, MN Department NURSING SERVICE (NURSV) Physical Location (building) BRAINERD MEDICAL CLINIC Auto req ID 10099BR Job Description The Clinic Care Coordinator acts as an access liaison, resource, educator, coach, advocate, navigator and partner for patients enrolled in the Primary Care Coordination Program. The Care Coordinator provides information about care coordination services, to patients and their families who have or are at risk for chronic or complex conditions. Working with the identified patients and their families, the Care Coordinator assists in the formulation of individualized Care Plans and pre-plans for productive provider visits. The Care Coordinator uses skill in assessment to identify care needs and communicate these needs to the care team. As a member of the care team, this position participates in process and performance improvement activities, assists the patient and family with care transitions, and facilitates partnership and ongoing communication with the patient's family.
Education: Graduate of an accredited nursing program with a minimum of two years nursing experience, including experience with chronic disease management, public health, case management and/or experience in an acute care setting. Bachelor's degree in Nursing strongly preferred.
Licensure/Certification: Currently licensed as a Registered Nurse in the State of Minnesota.
