Job Summary: The RN Case Manager will determine the medical necessity of all admissions and the need for continued stay.To identify inappropriate or untimely services, focusing on opportunities to reduce inappropriate patterns of utilization.Discharge planning assessment of all ages to begin in the pre-hospital phase, minimal upon admission to facility.To coordinate the delivery of services to meet patient's discharge needs.To be capable and comfortable offering alternatives in patient care (Home Health, SNF, ALF, etc.).To facilitate timely and appropriate discharge from the hospital.To identify opportunities for improvement in patient care.To provide documentation of all functions as required by regulatory agencies.
Essential Duties:
This position requires knowledge of Care Management policies and procedures and State and Federal regulations regarding medical necessity for admission and discharge screens. Knowledge of community resources and discharge planning, along with the technical skills necessary to demonstrate alternative care programs to patients and their families. Frequent interactive, interpretive and consultative associations are inherent to this position. The ability to retrieve, communicate, and present data and information verbally and in written form if required. Strong interpersonal skills and the ability to communicate effectively with patients, families, nursing staff, physicians, associations and community resource liaison are inherent to this position. A professional appearance is essential. The Case Management Coordinator must possess knowledge of Universal Precautions. The ability to maintain confidentiality of information is required.
Requirements:
- RN or LPN license required - Prefer minimum two years clinical experience in an acute care setting with a working knowledge and expertise in understanding diagnosis, medications, therapies and discharge planning, as they relate to patient care - Current knowledge of available health care providers, programs and community resources - Minimum of six months utilization management and discharge planning background preferred - Current State of Florida RN or LPN license - Participation in Continuing Education - Knowledge of Federal and State guidelines and requirements: HCF, Medicare, Medicaid, Champus and HRS - Assessment, communication, motivational and organizational skills - CPR certification preferred - Active participation in professional group to keep updated in changes in regulations
