New York State Cancer Services Program (NYS CSP) funded position accountable for in-reaching to AMS/GFH patients who are due for recommended cancer screenings, specifically colorectal, breast, and cervical screenings following the NYS CSP evidence based standards for preventive care. Develop and maximize electronic systems to determine baseline compliance with screening recommendations, to reach individual patients, to communicate with providers, and to support the provision of comprehensive care.
Education/Experience: Associates degree or higher and 3-5 years experience in medical practice administration and/or public health work required. Significant experience with IDX, MedEnt, Epic, or other practice management and electronic health records systems preferred. Skills/Abilities: Excellent verbal and written communication skills. Ability to accurately collect and record data. Ability to interpret data collection accurately. Ability to work with Microsoft office products. Ability to work independently. Ability to problem solve/trouble shoot within program activities.
