QI Facilitator - Medical Home
The patient-centered medical home (PCMH) is an approach to providing comprehensive primary care for children, youth and adults in a healthcare setting that facilitates partnerships between individual patients, their personal clinicians, and, when appropriate, the patient?s family. PCMH practices have a whole person orientation, and patients have a continuous relationship with a personal clinician. Care is integrated and coordinated, and quality and safety are hallmarks. Enhanced access is made possible by system advances and new communication options. What you?ll do:
You?ll collaborate with clinic leadership, quality improvement teams, and practice coaches to provide technical assistance on a variety of clinic operations redesign topics, such as enhanced access, team-based care, care coordination, and patient experience. Examples of typical accountabilities include:
? Planning and delivery of technical assistance programs; monitoring progress and outcomes of interventions. ? Conducting baseline assessments, analysis, and development of practice-specific technical assistance plans. ? Preparing and reviewing technical assistance support materials. ? Providing QI guidance to clinic teams and leadership; delivering formal presentations to facilitate the transformation toward PCMH attributes. ? Assisting with preparations to apply for NCQA patient-centered medical home recognition or other medical home designations.
Requirements:
Requirements include: ? At least three years of clinical quality improvement, process improvement, and/or population management experience. ? A clinical or healthcare operations background in primary care. ? A degree in health administration, health information, nursing, or public health.
Preference will be given to applicants with the following: ? Current, active nursing, nurse practitioner, or physician assistant licensure. ? Advanced degree in health administration, health information, nursing, or public health. ? Certification in healthcare quality or healthcare information. ? Familiarity with health information technology, including practice management systems, electronic health records, and/or registries. ? Experience working with safety net practices. ? Knowledge of the Patient-Centered Medical Home and/or Chronic Care models.
